A well-established skilled nursing facility in the Leominster, MA area is seeking an experienced RN MDS Coordinator to join our team. The ideal candidate will be detail-oriented, organized, and knowledgeable in MDS 3.0 processes and CMS regulations.

Responsibilities include:

Accurately completing and submitting MDS assessments and care plans Collaborating with interdisciplinary teams to ensure compliance and optimal resident outcomes Monitoring quality measures and ensuring accurate RUG scores Maintaining current knowledge of Medicare/Medicaid regulations

Qualifications:
