Location :

This position is for the North Shore. We have cases clustered around a small geographic area, which opens the opportunity for lots of patients without much travel, if you live close by.

Hours:

We are flexible with the hours that our nurses want to work. Per diem opportunities. Please note : Pay is calculated per visit, with differential pay rates, depending on the nature of work. Why work for

Position Summary:

In-home ongoing quality care.

Skilled Nursing:

Nurses help clients achieve wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, service facilitation, and use of evidence-based guidelines or standards which result in holistic and well measured care. Case notes are important. Nurses provide emotional support, reassurance and encouragement to patients. Responsibilities Provide in-home nursing care across community and home health settings, including patient assessment, vital signs monitoring, and medication administration per physician orders. Develop and implement individualized care plans, educate patients and families, and coordinate with interdisciplinary teams to ensure continuity of care. Perform procedures within scope (catheterization, IV insertion, basic life support, wound care) and manage equipment, EMR/EHR documentation, and charting. Monitor patient status, identify changes, escalate concerns, and provide psychological and emotional support as part of comprehensive care.

Pay:

$33.00 - $100.00 per hour
