Job Description
Summary:
The PT Nurse will provide quality support services to individuals as well as training to staff in Residential Services. This mainly consists of case management, and training supports in a group home setting. The ideal candidate will have experience working with the adult DD/ID population.Essential Duties & Responsibilities:
Assists individuals with activities of daily living which include but are not limited to; personal hygiene, self-care tasks, dressing. Maintain documentation in each individual's file of any medical or health related recommendations made at periodic reviews. Train staff in relevant medical and health care procedures and provide consultation to staff in regard to medical issues Assist staff in obtaining MAP certification and recertification Occasional administration of medication if VNA is not available Ensures the accurate and timely completion and submission of paperwork including but not limited to quarterly reports, medical forms and assessments. Serves as a liaison between the agency and medical professionals as needed. Advises leadership and staff of any medical problems that may hinder and individual's progress or that may be affected by activities at the program. Ensures the confidentiality of individual's medical records by complying with all HIPAA regulations. Other responsibilities as assigned. MA Nursing License (RN) At least 1 year in Community Health setting preferred Experience in DDS funded programs preferred MAP training experience, or willingness to become a certified trainer Valid Driver's License Covid-19 Vaccine Card Ability to pass background checks Prior experience working with the adult DD/ID population preferred Excellent written, verbal and interpersonal communication skillsWhat Walnut Street Center Offers :
