Schedule:

13-week full-time assignment, Monday through Friday On-call coverage may be required

Key Responsibilities:

Deliver skilled nursing care and interventions within patients homes Conduct thorough patient assessments and complete OASIS documentation accurately and promptly Collaborate with physicians and multidisciplinary teams to tailor individualized care plans Provide wound care, medication management, patient education, and monitor for changes in patient status Communicate effectively within the clinical team regarding updates or changes in condition

Qualifications:

Active RN license At least one year of recent home health RN experience Required experience with OASIS documentation Proficiency with HomeCare HomeBase (strongly preferred) Ability to manage a consistent, structured full-time schedule and meet daily visit targets Reliable transportation and a valid drivers license
