+ Developing, implementing, and evaluating individualized plans of care.

+ Providing skilled nursing care, education, and disease management.

+ Monitoring patient progress and identifying changes in condition.

+ Collaborating with physicians, therapists, caregivers, and interdisciplinary team members to coordinate care.

+ Educating patients and families on medications, treatments, and disease management.

+ Accurately documenting visits and maintaining timely clinical records.

+ Ensuring compliance with all clinical standards, agency policies, and HIPAA regulations.

+ Advocating for patients while promoting the highest quality of care.

  • Qualifications
  • + Current, unrestricted Massachusetts Registered Nurse (RN) license.

+ Minimum of one year of nursing experience in a clinical setting.

+ Home Health experience preferred, but not required.

+ Experience with OASIS documentation is preferred.

+ Experience with Homecare Homebase is a plus.

+ Strong assessment, communication, and organizational skills.

+ Associate's Degree in Nursing required; Bachelor's Degree preferred.

+ Valid driver's license, reliable transportation, and current auto insurance.

+ Ability to travel throughout the Newton service area.

+ Ability to safely lift, push, pull, and assist with up to 50 pounds as needed.
