From evaluating evaluates new patients, to outlining the care plan, you will determine the best care plan, most appropriate of services. you will conducts initial start-of-care clinical assessments, and establishes customized home health treatment plans. Key responsibilities include verifying Medicare or insurance eligibility, completing OASIS assessments, and performing medication reconciliation during the transition from hospital to home.
Key Responsibilities Patient Assessments:
Perform comprehensive physical head-to-toe evaluations and review medical histories in the patient's home.
Start of Care & OASIS:
Complete official start-of-care paperwork and electronic OASIS data sets required for home health compliance. •
Care Planning:
Design initial patient-specific care plans, set visit frequencies, and secure physician approval. •
Medication Reconciliation:
Review active prescriptions, identify potential drug interactions, and document safety protocols. •
Patient Education:
Instruct patients and families on disease management, equipment use, and self-care steps. •
Care Coordination:
Collaborate with intake teams, physical therapists, social workers, and doctors. •
Care Planning:
Design initial patient-specific care plans, set visit frequencies, and secure physician approval. •
Medication Reconciliation:
Review active prescriptions, identify potential drug interactions, and document safety protocols. •
Patient Education:
Instruct patients and families on disease management, equipment use, and self-care steps. •
Communication:
Compassionate listening and clear guidance for families during stressful health transitions. •
Critical Thinking:
Fast identification of home environment safety risks or complex medical needs. As an Admissions Registered Nurse Coordinator you will: Complete Start of Care (SOC) visits each day Conduct comprehensive patient assessments and accurately complete OASIS documentation Develop individualized plans of care in collaboration with physicians and the interdisciplinary care team Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education Educate patients and caregivers on disease management, medications, and home safety Monitor patient conditions and communicate changes appropriately Ensure documentation is accurate, timely, and compliant with regulatory standards •After sending resume, TEXT to 860 930 8781 and we will confirm receipt. Pay:
$110,000.00 - $120,000.00 per year
