Hospice RN Case Manager Hospice Services of Massachusetts - 3.1 Rockland, MA Job Details Part-time | Full-time Up to $51 an hour 7 hours ago Benefits Mileage reimbursement Paid holidays Dental insurance Vision insurance 401(k) matching Employee discount Opportunities for advancement Life insurance Qualifications CPR Certification RN License BLS Certification Full Job Description As a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families. Prior hospice experience is preferred, but not required. Full-time and part-time openings to cover Plymouth County and portions of Bristol County. Weekly pay up to $51 an hour.
Responsibilities:
Supervise hospice aides Complete comprehensive assessments of patient and family to determine hospice needs Provide professional nursing care by utilizing all elements of the nursing process Develop and implement individualized care plans Collaborate with interdisciplinary team members to ensure comprehensive care Provide health care instructions to patients and families Counsel patients and families on meeting nursing and related needs
Requirements:
Graduate of an accredited school of nursing Must possess an active and unencumbered RN license in Massachusetts Prior hospice experience preferred CPR certified
