Hospice RN Case Manager M-F days / Full Time & Part Time In this role, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families.

Responsibilities:

Supervise hospice aides Complete comprehensive assessments of patient and family to determine hospice needs Counsel patients and families on meeting nursing and related needs

Requirements:

Graduate of an accredited school of nursing Must possess an active and unencumbered RN license in Massachusetts Prior hospice experience preferred or home care .

Pay:

Up to $51.00 per hour Expected hours: 24.0 - 40.0 per week
