Position:
Residential RN Healthcare Coordinator Location:
Wakefield, MA Pay rate: $68,640 annual salary
Hours:
Full-time, 30 hours per week
Schedule:
This position will typically operate on a Monday - Friday schedule; may be required to work evenings and weekends in emergency healthcare needs; must have flexibility as schedule may vary to meet needs of agency and the Individuals we support Benefits of Working for us! Incremental Pay increases based on years of service, pending performance evaluations 3 weeks' vacation, 1 week sick time, and 12 holidays for new full-time employees Low-cost benefit plans - Medical & dental insurance, employer-paid life insurance, and long-term disability coverage Tuition reimbursement eligibility after 90 days of employment Summary & Responsibilities The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities. Staff training on specific treatment protocols Consultation for individual medical needs Perform MAP trainings and recertifications Perform MAP and medical file audits Residential home and hospitalization visits as needed Attend administrative meetings and complete related paperwork Qualifications Must have a current Massachusetts license as a Registered Nurse (RN). Must have at least 3 to 5 years' experience in a community health setting; preferably with adults with developmental and intellectual disabilities. Certified MAP Trainer is a plus but training is also available Successful candidates will be required to complete a background check including references, CORI, and fingerprinting.
