PM What Youll Bring:

Active RN license in good standing for the state of practice At least 1 year of recent, hands-on Home Health experience (field experience strongly preferred) Demonstrated proficiency with Home Care Home Base (HCHB) documentation system Experience conducting OASIS assessments Reliable transportation, current drivers license, and auto insurance

What Youll Do:

Travel daily within an assigned territory to complete approximately 35 patient visits per day (caseload targets 30 points per week) Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing care, including wound care, medication administration, IV therapy, and management of chronic conditions Closely monitor patient progress and report any notable changes to the physician and care team Provide patient and family education to support ongoing health and recovery Complete timely, accurate documentation in HCHB Collaborate with a multidisciplinary team including therapists, social workers, and aides for coordinated patient care Participate in regular case conferences, staff meetings, and ongoing educational activities Maintain compliance with all regulatory and agency standards
