Rotating Weekends Pay:

$45-$53/hour Make an Impact Beyond the Bedside As a Case Manager , you'll serve as a critical connection between patients, families, providers, payors, and the interdisciplinary care team. You'll help ensure patients receive the right care at the right time while supporting a smooth transition from admission through discharge. What You'll Do

• Coordinate patient care and collaborate with an interdisciplinary team
• Develop and monitor individualized treatment and discharge plans
• Meet with patients and families to establish goals and communication plans
• Facilitate weekly team conferences and family meetings
• Coordinate referrals and post-discharge services
• Monitor patient progress, quality, utilization, and payor requirements
• Support utilization review and identify opportunities to improve outcomes
• Help prevent discharge delays by coordinating resources and services
• Ensure accurate and timely case management and discharge documentation
• Educate patients and families throughout the rehabilitation process
• Participate in acute-care transfer reviews and help identify trends
• Stay current on applicable regulations, standards, policies, and payor requirements What You Bring Active license/certification appropriate to your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling, etc.

) Ability to independently complete assessments within your scope of practice RN candidates: Minimum Associate Degree in Nursing; BSN preferred Other eligible healthcare professionals: Minimum Bachelor's degree; graduate degree preferred Eligible for CCM® or ACM™ certification upon hire or able to obtain certification within 2 years 2+ years of rehabilitation experience preferred Benefits & Perks

Pay Details:
