Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical history, medical conditions, and medication records Educate patients regarding conditions, medications, and overall health management Identify and document care gaps, sharing actionable recommendations Focus exclusively on assessmentno treatment planning or medication prescribing

Preferred Qualifications:

Active Nurse Practitioner licensure in Maryland Board certification via AANP or ANCC Current CPR certification (BLS required) Completed MSN degree from an accredited nursing program At least 6 months of recent NP clinical experience (strongly preferred) Enjoy competitive pay for each completed assessmentbetween $80 to $90 per evaluation, with an added mileage stipend and compensation for administrative work.
