MDS Coordinator Hallmark Health Consulting Services is seeking a highly motivated and organized MDS Coordinator to join our team in Baltimore, MD . Our dedicated team of healthcare professionals works together to provide individualized care plans that enhance the quality of life for our patients. About the
Role:
As a MDS Coordinator , you will play a critical role in ensuring the accuracy and completeness of the Minimum Data Set (MDS) assessments for our patients. Your responsibility will be to coordinate the MDS process, including reviewing medical records, gathering necessary data, and submitting completed assessments to the facility. You will also be responsible for communicating with healthcare providers, nurses, and other team members to ensure the successful implementation of care plans.
Responsibilities:
Coordinate the MDS assessment process, ensuring timely and accurate completion of assessments Review medical records, gather necessary data, and submit completed assessments to the facility Communicate with healthcare providers, nurses, and other team members to ensure successful implementation of care plans Collaborate with the care team to develop and implement individualized care plans Maintain accurate and up-to-date records of patient assessments and care plans Participate in quality assurance and quality improvement initiatives to ensure continuous improvement of patient care
Requirements:
High School Diploma or equivalent required; Post-secondary education in a related field preferred Strong organizational and communication skills Ability to work effectively in a fast-paced environment Familiarity with electronic medical records (EMRs) and MDS software a plus
