Your nursing expertise will make a difference in the lives of the clients and communities we serve. Along with home visits and assessments for personal care and dementia-related care, you will participate in supervision of personal care providers as well as educate clients, loved ones and family members.

Responsibilities:

Perform the initial home care assessment in the client's home Explains all home care service policies to the client, including rates of service, billing options, HIPAA Privacy; and obtains signature of acknowledgment Based on the needs assessment and in conjunction with the client/family, completes an individualized Care Plan Completes all initial paperwork and initiates the Client Registration process Coordinates home care services and care plan implementation with the care team Teaches the client and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician Recognizes and utilizes opportunities for health counseling with clients and family/caregivers Addresses client questions/concerns to ensure client satisfaction Supervises ancillary personnel and delegates responsibilities when required Promotes home care services through regular community contacts and follow-up with referral sources Participates in marketing and community outreach activities as requested

Qualifications:

Currently licensed as a registered nurse in Maryland with no previous disciplinary measures Previous experience in nursing or other medical fields Current BLS or CPR/First Aid certification Possess excellent observation, good nursing judgment, and good oral and written communication skills Is self-directed with the ability to work with little supervision and has good organizational skills is flexible and cooperative in fulfilling role obligation Ability to build excellent rapport with clients, staff, and family Strong problem solving and critical thinking skills Ability to work in a fast-paced environment independently and as part of a team We offer mileage reimbursement We offer 401K plan through Paychex About Trusted Hearts Homecare Solutions LLC Trusted Hearts Homecare Solutions is a trusted community leader that provides in-home care.
