Key Responsibilities:
Conduct annual home health risk assessments for Medicare Advantage plan members in locations across Harford County, including Aberdeen, Bel Air, and Edgewood Assess prior and current medical history, ongoing medical conditions, and medication regimens Offer patient education on health conditions and prescribed medications Identify gaps in care, reporting findings and providing actionable recommendations No responsibility for treatment planning or prescribing medications
Qualifications:
Active Nurse Practitioner License in the state of Maryland Current CPR (BLS) certification Board Certification through AANP or ANCC required MSN degree from an accredited nursing program Minimum of 6 months experience as a Nurse Practitioner is preferred Benefit from competitive compensation at $80$90 per evaluation, alongside a mileage stipend and additional administrative time reimbursement.
