Job Description

JOB DESCRIPTION DETAILS
Job Summary:
Performs utilization review and discharge planning to patients through the continuum of care. Creates care plans for patients, updates the care plans according to the patients' conditions, and educates patients and their significant others on how to properly follow the care plans.
Education:
Position requires an associate's degree (A.A) in Nursing. BSN or bachelor's degree in a related field, preferred.
Registration/Certification/Licensure:
Registered Nurse (RN) License from State of Maryland or Compact State preferred. Licensed Practical Nurse (LPN) License acceptable with case management experience.
Experience:
Position requires a minimum of 3 years experience performing direct nursing care in an acute care setting or case management.
Other Requirements:
Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. Effectively communicates with all patient populations and staff by implementing organization-wide communication techniques as a daily practice. Effectively advocates for quality patient safety and satisfaction as a daily practice. Ability to effectively use computer applications to perform job duties and responsibilities. Ability to work autonomously and effectively prioritize workload. Ability to communicate professionally, thoroughly, and accurately both verbally and in writing.
