Job Description

Incredible Health is seeking a compassionate, detail-oriented Registered Nurse to deliver exceptional patient care within our partner healthcare & medical services network. In this role, you'll assess, plan, implement, and evaluate patient care, collaborate with interdisciplinary teams, and educate patients and families. You'll use evidence-based practices, follow safety and quality standards, and document accurately in electronic health records. Ideal candidates are mission-driven, adaptable, and comfortable working with technology-driven tools that improve staffing, clinician experience, and patient outcomes nationwide. Responsibilities Perform comprehensive and focused patient assessments and develop individualized plans of care Administer medications, treatments, and therapies safely and in accordance with provider orders and facility protocols Monitor patient status, interpret clinical data, and respond promptly to changes in condition Document all assessments, interventions, and outcomes accurately in the Electronic Health Record (EHR) Educate patients and families on diagnoses, medications, procedures, and post-discharge care plans Collaborate with physicians, advanced practice providers, and interdisciplinary team members to coordinate safe, efficient care Adhere to infection control standards, safety policies, and regulatory requirements Participate in quality improvement initiatives and contribute to optimizing clinical workflows Utilize digital and technology-driven tools provided through Incredible Health's platform to enhance communication, scheduling, and care coordination Support smooth patient transitions between levels of care and care settings, including discharge planning Required Skills Patient assessment Care planning Medication administration Electronic Health Records (EHR) documentation IV insertion and management Wound care Care coordination Clinical triage Patient and family education Infection control Vital signs monitoring Time management Clinical decision-making Interdisciplinary collaboration Basic life support Advanced cardiac life support (if applicable) HIPAA compliance Quality and safety reporting Use of digital health platforms Telemetry monitoring (if applicable)
