Base salary + Company Car | Washington, DC & Montgomery County, MD Job description: We are seeking a dedicated and compassionate Nurse Case Manager (RN) to join our growing home care team. This full-time Nurse Case Manager (RN) position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Washington, DC and Montgomery County, MD communities. Friday Work Schedule:

9:00 AM - 5:00

PM RN Case Manager Key Responsibilities:

Conduct initial client assessments and routine reassessments in home settings Conduct hospital discharge assessments Develop and update individualized care plans Collaborate with caregivers, families, and interdisciplinary teams Monitor client progress and ensure compliance with care standards Maintain accurate and timely documentation Completing annual skills assessments and competencies for LPNs and CNAs/Caregivers, Training and supervising healthcare personnel: LPNs, CNAs, and Caregivers. Attend dischange planning as needed

Qualifications:

Active Maryland and DC RN license in good standing (Compact/Multi-state) At least two years of clinical experience (home care, case management, or patient assessment) Strong clinical judgment and communication skills Ability to work independently and manage a flexible schedule

Current CPR Certification, AHA BLS What We Offer:
