Job Description

Take on a rewarding opportunity as a healthcare professional specializing in home health risk assessments for Medicare Advantage plan members throughout Harford County. As a Nurse Practitioner in this per diem role, you will use your expertise to enhance patient care while enjoying flexible scheduling and autonomy.
Schedule/Hours:
Flexible shifts available Monday through Sunday Design your own schedule to suit your lifestyle Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
Conduct comprehensive annual home health risk assessments during 45-minute in-home visits Evaluate medical history, current conditions, and medication profiles Deliver patient education regarding health conditions and medications Identify care gaps and suggest actionable recommendations for improved health outcomes Visits may include both annual assessments and post-discharge follow-ups No requirement to create treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner License (MD) Board certified via AANP or ANCC Current BLS (CPR) certification Minimum of 6 months experience as a Nurse Practitioner preferred MSN from an accredited institution In this role, you will receive competitive compensation of $80-$90 per evaluation, in addition to a mileage stipend and reimbursement for administrative time.
