Traveler Requirements:

Active Maryland RN license; minimum 2 years of hospital inpatient RN Case Management/discharge planning experience; Med-Surg and/or Critical Care discharge planning experience; BLS; strong inpatient discharge planning experience; able to work independently after limited orientation; experience coordinating discharge needs, resources, and transitions of care.

Responsibilities:

Support adult and geriatric inpatient populations in the Emergency Department and medical inpatient units. Partner with Social Work and other healthcare professionals to coordinate discharge planning and patient transitions. Manage medically complex Med-Surg and Critical Care patients, ensuring safe and effective discharge processes. Support approximately 1.5-2 units during crisis staffing, supporting multiple inpatient units as needed. Chart patient progress and discharge details using Cerner system, maintaining accurate documentation. Why Choose VyTalent Solutions?
