Key Responsibilities:

Conduct in-home annual risk assessments for Medicare Advantage members Review, evaluate, and document current and prior medical history, conditions, and medication usage Educate patients regarding their medical conditions and appropriate medication usage Identify care gaps and provide recommendations for follow-up No requirement to create treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner license in Maine Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months current experience as an NP preferred MSN degree from an accredited nursing program Per evaluation, you will receive compensation between $80 and $90, plus mileage stipend and administrative time, offering you flexibility and fair reward for your expertise.
