Director of Operations Position Overview:
The role of a Registered Nurse (RN) at Circle of Hope is an essential member of the Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) who is responsible for supporting the clinical team in the delivery of safe, effective, and compassionate mental health services, supporting clients with severe mental health disorders, and working closely with a multidisciplinary team to achieve optimal treatment outcomes. The RN works closely with the clinical, medical, and administrative staff to ensure coordination of care and ongoing medical support to participants during program hours.
Key Responsibilities:
The Registered Nurse (RN) duties include, but are not limited to:
Clinical and Medical Care:
The RN will complete intake referral packets accurately and thoroughly as part of the intake process. The RN understands that timely and comprehensive intake documentation, screening, and referrals are essential for effective treatment planning and continuity of care. The RN will conduct initial nursing assessments upon client admission, including vital signs, health history, and medication reconciliation. The RN understands that comprehensive screening and accurate assessment are crucial for effective treatment planning and referral to other services when necessary. The RN will implement the physician's orders promptly with an understanding of the purpose of each order as it relates to the client's plan of care. The RN will monitor and document client' physical and emotional well-being during program hours. The RN agrees to monitor and manage side-effects, medication compliance, and coordinate care with the prescribing provider. The RN will respond to medical emergencies and psychiatric emergencies using appropriate protocols. The RN will screen clients for mental health issues, substance use, and other psychosocial factors that may affect their treatment, referring to outside medical services when indicated. The RN will ensure that all assessments and evaluations are completed in accordance with facility policies and procedures, including required timelines for submission and updates.
Treatment Planning:
The RN will initiate discharge planning at the start of treatment, identifying potential barriers to successful transition, coordinating with the treatment team to ensure continuity of care upon discharge, and providing relevant medical input. The RN will collaborate with the treatment team to ensure the care provided is individualized and appropriate to the client's diagnosis and level of functioning. The RN will develop, implement, and review individualized treatment plans, ensuring that these plans align with each client's therapeutic goals. The RNwill collaborate with clinical and medical providers to adjust nursing interventions based on ongoing assessments. The RN will attend regular treatment plan meetings, and ensure the client is present, as well as adjust treatment strategies based on client progress to maximize client functioning and enhance departmental performance. The RN will ensure that all treatment planning and case management documentation is completed in accordance with facility policies and procedures, including required timelines for submission and updates.
Nursing Interventions:
The RN will provide client care by conducting group therapy and/or participating in family therapy, as well as engaging in crisis intervention as part of my nursing responsibilities. The RN will provide direct, evidence-based nursing interventions to address clients' physical and behavioral health needs, including: Monitoring for signs of withdrawal or medication reactions and implementing appropriate interventions. Providing wound care, infection control measures, or other basic medical care within scope. Implementing de-escalation techniques and supportive communication during mental health crises. Educating clients on medication adherence, chronic health conditions, symptom management, and health promotion. Offering supportive counseling within the nursing scope to reinforce coping strategies, treatment goals, and safety plans. The RN agrees to implement physician orders and communicate changes in clinical status promptly. Client & Family Engagement (only with a Release of Information (ROI) or a Legal Guardian): The RN will provide supportive, respectful, and culturally responsive care to all clients and their families. The RN will educate clients and family members about diagnoses, medications, treatment options, prognosis, and health-related issues in an accessible and compassionate manner. The RN will promote health literacy, engagement in treatment, and shared decision-making wherever possible.
Documentation and Recordkeeping:
The RN is required to complete all mandatory client documentation, including but not limited to intake documentation, admission documentation, medication documentation, treatment planning documentation, nursing note documentation, incident reports, discharge documentation, and any other required clinical documentation in a timely and professional manner. The RN will maintain accurate and up-to-date client records, including paper medical records and electronic medical records (EMR), ensuring compliance with state, federal, and facility regulations. The RN will ensure that all entries in the paper medical record and electronic medical record (EMR) are clinically sound, reflect the services provided, and meet auditing and billing requirements. The RN understands the importance of timely documentation and will adhere to all policies and procedures related to documentation timelines, confidentiality, and privacy standards.
Collaboration & Communication:
The RN agrees to maintain a functional rapport with all personnel employed by Circle of Hope, as well as anyone that the organization collaborates with. If the RN is unable to continue functional rapport with an individual, the RN will immediately notify the Director of Operations. The RN will work as part of a multidisciplinary team, collaborating with Psychiatrists, Nurses, Social Workers, and other healthcare professionals to deliver high-quality care to clients. The RN understands that collaboration is not limited to direct care but includes participation in regular team meetings, client care meetings, clinical discussions, case consultation, and potential staff interviews. The RN is required to actively participate in direct care, regular team meetings, client care meetings, client assessments, clinical discussions, and discharge planning. The RN will contribute to treatment planning and care coordination to ensure continuity and high-quality of care across disciplines. The RN may be asked to assist in orienting new staff to the program by sharing clinical processes and program expectations. When needed, the RN will provide peer consultation to support clinical decision-making and promote team development. The RN agrees to communicate effectively with the clinical team, medical providers, and external care teams. The RN understands that additional responsibilities may be assigned on an ongoing basis, based on organizational needs and priorities, beyond those outlined above.
Performance Improvement:
The RN will actively participate in departmental quality assessments and quality improvement initiatives, including setting goals, collecting and evaluating relevant data, and assisting with the development and implementation of action plans to improve service delivery and client outcomes. The RN will subject my clinical case activity and documentation to supervisor and peer review as part of ongoing performance improvement efforts and to support quality assurance measures.
Compliance and Ethical Standards:
The RN agrees to uphold the highest standards of ethical and professional conduct, including: Complying with all federal, state, and local laws and regulations governing nursing practice and mental health services. Following all policies and procedures of the organization, including those related to clinical documentation, medication management, client rights, and incident reporting. Always maintaining professional boundaries with clients, avoiding dual relationships and conflicts of interest. Practicing within the scope of a Registered Nurse license and seeking guidance when uncertain about clinical or ethical decisions. Reporting any observed or suspected violations of policy, law, or professional standards to the appropriate supervisor or compliance officer. Participating in mandatory trainings related to compliance, ethics, confidentiality, and safety. Upholding principles of equity, cultural sensitivity, and trauma-informed care in all interactions. The RN understands the importance of continuing education to stay current with best practices and state regulations, as well as continuing professional growth and development.
Crisis Management:
The RN will respond promptly and professionally to crisis situations, ensuring the safety and well-being of clients at all times. The RN will conduct risk assessments, including evaluations for suicidal ideation, self-harm, harm to others, or significant deterioration in mental status. The RN will assess the potential need for a higher level of care, including inpatient hospitalization or emergency psychiatric services, and coordinate referrals when clinically indicated. The RN will develop and implement safety plans in collaboration with clients, families, and the treatment team. The RN will document all crisis interventions and assessments thoroughly, following facility procedures and clinical standards.
Qualifications:
Education:
The RN has an Associate Degree in Nursing (ADN), a Bachelor of Science in Nursing (BSN) degree, or a Master of Science in Nursing (MSN) degree from an accredited higher education institution.
Licensing:
The RN holds a Registered Nurse (RN) license in the State of Michigan.
Experience:
The RN has at least two years of experience in nursing, particularly in mental health settings, and is familiar with evidence-based practices.
Skills:
1) The RN possesses the ability to understand a variety of communication forms, such as written, oral, diagram, or schedule form, 2) The RN demonstrates the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and policy and procedures manuals, 3) The RN is proficient in maintaining accurate documentation and working collaboratively within a team, 4) The RN has the skills necessary for effective nursing assessments, treatment planning, and crisis intervention, 5) The RN possesses the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists, 6) The RN demonstrates the ability to interact with people of various backgrounds in a courteous, tactful, and poised manner, & 7) The RN has knowledge of occupational health and safety, workers compensation, and OSHA regulations.

Sedentary Work:
Exerting up to 10 pounds of force occasionally. Involves sitting most of the time, though may involve walking or standing for brief periods. Frequent use of hands and fingers for typing, writing, or handling paperwork.
Light Work:
Exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently. May involve walking or standing to a significant degree, or working at a desk while performing some lifting or carrying. May involve pushing/pulling of objects or rolling carts. Requires frequent use of arms and hands for reaching, lifting, and fine motor skills.
Medium Work:
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. Requires standing and walking for extended periods. Involves frequent lifting, carrying, pushing, or pulling of materials or equipment. May include bending, kneeling, or stooping.
