Guaranteed base Salary, Per point incentive pay in addition to the base salary, Per point value between $55 and $58.60, Earning Potential of $3,461

•$3,500+ bi-weekly, Payroll processed bi-weekly (26 pay periods), Mileage reimbursed at $0.55/mile.

Shift Type:

Full-time, Monday

•Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities:

Assumes responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care. Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. Participate in on-call rotation as reasonably assigned.

Qualifications:

Minimum of 1 year of nursing experience with a preference for 2+ years in an acute or a post-acute care setting. Active State Registered Nurse licensure is required. Proof of current CPR certification and valid Driver's license.
