Our organization is seeking qualified Registered Nurses (RN) to work with our geriatric population in their own homes. Responsibilities include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
