The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review . Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

Be a referral-source ambassador:

every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

Set the standard others copy:

your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. What you'll own Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge). Direct, high-acuity patient care, with a focus on wound care Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases. Being the face of Adaptive to patients, families, and the referral sources who feed your pod. Who you are / how you operate Dedicated clinician: home health experience strongly preferred High ownership: you treat your caseload like your own panel and your pod like your own business.
