Prefer not to record during a patient visit? No problem. Our platform also includes an after-visit voice documentation option , allowing you to capture your notes afterward and leverage AI to draft your charts. By streamlining documentation and reducing after-hours charting , our technology helps you spend less time on paperwork and more time doing what you do best, providing exceptional patient care. The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review . Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador:
every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy:
your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. What you'll own Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge). Direct, high-acuity patient care, with a focus on wound care Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases. Being the face of Adaptive to patients, families, and the referral sources who feed your pod. Who you are / how you operate Dedicated clinician: home health experience strongly preferred High ownership: you treat your caseload like your own panel and your pod like your own business.
