Job Description

Phone Triage Nurse Job Type:
Part-time The Phone Triage Nurse provides safe, timely and compassionate telephone triage for patients of the practice. This is the perfect opportunity for an experienced nurse looking to utilize their critical thinking, symptom evaluation and patient communications skills in a daytime clinic setting.
Essential Duties and Responsibilities:
  • Answer incoming clinical phone calls and patient portal messages.
  • Assess symptoms using approved triage protocols and nursing judgement.
  • Determine appropriate level of care, including home care, office appointments, urgent evaluation or emergency services.
  • Schedule same-day and follow-up appointments as appropriate.
  • Document all patient interactions accurately in EMR.
  • Route complex clinical concerns to providers for review.
  • Process Prescription refill requests.
  • Review laboratory and diagnostic results as directed by providers.
  • Provide patient education regarding medications, treatment plans and preventative care.
  • Coordinate follow-up care and communication with pharmacies and specialists.
  • Complete nurse visits, administer vaccines, urine cultures, stitches removal, weight checks etc.
  • Participate in quality improvement initiatives and cross-training.
Qualifications:
  • Licensed RN or LPN in the State of Michigan required
  • 5 years of primary care medicine and/or telephone triage preferred
