First Shift, Full Time (40 hours/week) Overview Bronson at Home is seeking a compassionate, skilled Registered Nurse to provide high-quality, patient-centered care in the home setting. This role delivers nursing services based on a physician-ordered plan of care and supports patients and families throughout their care journey. RNs work independently in the field while collaborating closely with the Home Health Clinical Services Manager and interdisciplinary team. What You'll Do Provide skilled nursing care according to physician-prescribed plans and Medicare guidelines. Assess, plan, implement, and evaluate individualized patient care. Coordinate care with physicians, therapists, and other team members. Delegate appropriate tasks to patient care assistants and support staff. Deliver hands-on nursing care based on patient needs and acuity. Document each visit, including patient responses and ongoing needs. Communicate effectively to support continuity of care. Demonstrate competency in caring for the populations served. Qualifications Diploma, Associate's, or Bachelor's degree in Nursing required. Active RN license in good standing with the State of Michigan required. BLS certification required. Two years of recent continuous acute care experience preferred. Driving Requirements Regular use of a personal vehicle for work-related duties. Valid Michigan driver's license and proof of auto insurance required. Successful MVR check required prior to start date. Ongoing responsibility to maintain and update proof of insurance. Knowledge, Skills & Abilities Strong assessment, planning, and clinical decision-making skills. Excellent communication skills (verbal, written, electronic). Ability to interpret clinical and electronic data. Comfortable with sustained computer use and detailed documentation. Ability to lift up to 50 pounds and assist with patient care activities. Professional Practice Expectations Deliver evidence-based, patient-centered care. Document care accurately and thoroughly. Collaborate with patients, families, and interdisciplinary teams. Participate in quality improvement, education, and professional development. Provide culturally sensitive, ethical, and compassionate care. Advocate for patients and contribute to a positive work environment. Role Responsibilities Collect and analyze comprehensive patient data. Identify patient needs and develop individualized care plans. Implement interventions, administer medications, and provide education. Evaluate patient progress and adjust plans as needed. Coordinate discharge planning and support transitions of care. Delegate tasks appropriately based on patient needs and staff competencies. Participate in peer review, research activities, and professional development. Maintain professional records and uphold Bronson's standards of practice. Physical & Cognitive Requirements Frequent computer use requiring sustained attention. Regular lifting of up to 50 pounds; occasional assistance with heavier tasks. High levels of mental and visual concentration. Work Environment Independent field-based nursing supported by a collaborative team. Participation in unit meetings, committees, and professional activities encouraged.
