Part-Time, with Mileage Reimbursement About Us Community Hospice provides compassionate, patient-centered end-of-life care to individuals and families throughout Manistee and Mason Counties. Our interdisciplinary team delivers physical, emotional, and spiritual support so patients can experience comfort, dignity, and quality of life in their final journey — wherever they call home. Position Summary We are seeking a licensed Registered Nurse to join our hospice interdisciplinary team (IDT) on a part-time basis, providing skilled nursing care to patients and support to families throughout Manistee and Mason Counties. This role involves managing a patient caseload, conducting home visits, and coordinating care with physicians, social workers, chaplains, and other IDT members to ensure comfort-focused, individualized care plans. Key Responsibilities Conduct comprehensive nursing assessments for hospice patients in home, assisted living, and facility settings Develop, implement, and update individualized plans of care in coordination with the interdisciplinary team Manage symptom control and pain management in accordance with physician orders and hospice best practices Provide skilled nursing visits based on patient acuity and plan of care frequency plans. Educate patients and family caregivers on disease progression, medication management, and comfort care techniques Participate in IDT care planning meetings, contributing clinical updates and recommendations Complete timely, accurate documentation in compliance with Medicare Hospice Conditions of Participation and state regulations Provide on call/after-hours support and respond to patient/family needs as scheduled Collaborate with social work, chaplaincy, and hospice aides to deliver holistic end-of-life care Pronounce death and support families through the immediate post-death period as needed Qualifications Current, unrestricted RN license in the State of Michigan 1+ years of nursing experience required; hospice, home health, or oncology experience strongly preferred CPR certification required Valid driver's license and reliable transportation for home visits across Manistee and Mason Counties Strong assessment, communication, and time-management skills Comfortable working independently in home settings with regular IDT collaboration Ability to participate in on-call rotation as required What We Offer Competitive hourly compensation: $40 / hour Mileage reimbursement for travel across Manistee and Mason Counties Flexible part-time schedule Meaningful work with a mission-driven organization providing end-of-life care Collaborative interdisciplinary hospice team environment
