What You'll Do Deliver one-on-one, high-impact skilled nursing care in the home setting and senior living facility settings (examples: Independent Living, Assisted Living). Practice in a senior-living-centric care model, supporting residents where they live and ensuring care aligns with the community environment and routines. Collaborate with senior living wellness teams and executive teams to drive better health outcomes for residents through shared goals, clear communication, and consistent follow-through. Collaborate with Community Health Navigators to streamline referrals, close loops on care plans, and ensure residents and families have a seamless experience. Complete accurate, timely assessments and documentation, driving high-quality care plans and safe transitions. Provide education that sticks—medications, disease management, warning signs, and prevention strategies. Coordinate care across disciplines (therapy, providers, families) with strong clinical judgment and a calm, reliable presence. What We're Looking For Take ownership and follow through.
