Role Overview As a Case Manager, you will be a vital member of the patient-centered care team, responsible for identifying, assessing, planning, and coordinating care to meet the health needs of individuals and families. What You Will Do Collaborate with the healthcare team to identify and assess patient needs. Develop and implement care plans that promote patient safety and quality of care. Coordinate services and resources to support patients' health outcomes. Provide effective discharge planning to ensure a safe transition to the next level of care. Advocate for patients and families, addressing their needs and concerns. Evaluate care plans and outcomes to optimize health along the care continuum. Qualifications What You Will Need Required Qualifications Bachelor's degree in nursing or a Master's degree in Social Work. Minimum 3 years of clinical experience. Excellent verbal communication and written documentation skills. Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Preferred Qualifications Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC). Experience in discharge planning, home health care, rehabilitative medicine, or managed care.Benefits
