Key Responsibilities Provide professional nursing care using the nursing process (assessment, planning, implementation, evaluation). Deliver services in clinic, home and community settings, including caring for pregnant individuals and infants under the age of one and their families. Provide health education, counseling, and suport to clients and families Assist clients in accessing healthcare and community resources Collaborate with internal teams, healthcare providers and community partners Maintain accurate, timely, and confidential documentation in compliance with HIPAA and agency standards Participate in outreach, training, and community health initiatives Regular travel within the service area
