On-Call Triage Nurse (RN)
Pay:
$40/hour + On-Call Stipend We are seeking an experienced Registered Nurse (RN) to join our team as an On-Call Triage Nurse . This position provides after-hours clinical support to Assisted Living communities by triaging resident concerns, guiding caregiving staff, and coordinating care to ensure resident safety and quality outcomes. Responsibilities Receive and triage calls from Assisted Living staff, including CNAs, CMAs, Medication Technicians, and other caregivers. Assess resident needs using nursing judgment and provide appropriate clinical direction. Communicate significant changes in resident condition to physicians, nursing leadership, and responsible parties. Support care coordination and individualized resident care plans. Respond calmly and professionally to urgent and emergency situations. Document assessments, incidents, interventions, and communications accurately. Review and process physician orders and medication changes as needed. Qualifications Active Registered Nurse (RN) license in the state of practice. Graduate of an accredited nursing program. Minimum of 2 years of nursing experience required. Assisted Living, long-term care, or dementia care experience preferred. Strong assessment, critical thinking, communication, and documentation skills. Ability to work independently and make sound clinical decisions.
