Our mission is to enhance quality of life by delivering expert, patient-centered care that supports both patients and their families during life's most important moments. Position Overview We are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10-15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life. This position offers a consistent Monday-Friday schedule, manageable caseload, and a localized territory in the northwest metro, allowing you to build meaningful relationships with patients and families.
What You'll Do Patient Care & Case Management:
Conduct initial and ongoing patient assessments Develop and manage individualized hospice care plans Administer medications and provide palliative interventions Educate patients and families on symptom management and care needs Provide emotional support to patients and families
Care Coordination:
Manage and coordinate care for assigned patients Collaborate with physicians and interdisciplinary team members Participate in care conferences and family meetings Advocate for patient needs and ensure access to resources
Clinical & Operational Excellence:
Maintain accurate and timely clinical documentation Supervise and support hospice aides Ensure necessary equipment and supplies are in place Participate in on-call rotation (5 days/month) Contribute to quality improvement initiatives Qualifications Active RN license in Minnesota Graduate of an accredited School of Nursing CPR certification 1+ year of acute care experience required Hospice or home care experience preferred Strong assessment and clinical decision-making skills Valid driver's license and insured vehicle Why Join Suite Living Hospice Care?
