Job Responsibilities Deliver Personalized Resident Care Conduct initial nursing assessments of prospective residents and ongoing reassessments in accordance with Minnesota Assisted Living Regulations. Develop, implement, and update individualized care plans and service agreements annually and as needed. Monitor resident health, safety, and continued appropriateness of placement, recommending alternative care arrangements when necessary. Communicate changes in resident conditions and care needs to physicians, staff, families, and other appropriate parties. Coordinate resident services with physicians, case managers, and other healthcare providers. Maintain accurate and timely resident records, assessments, nursing notes, and required documentation. Ensure staff understand and appropriately implement individualized resident care plans. Provide clinical guidance and respond to medical questions or emergencies, including participation in assigned on-call coverage. Oversee Safe Medication Practices Oversee medication management and administration practices to ensure compliance with physician orders, facility policies, and applicable regulations. Coordinate medication orders, renewals, and necessary follow-up with physicians. Ensure physician orders are signed and implemented within 24 hours in accordance with facility procedures. Set up resident medications according to individualized care plans and applicable nursing standards. Monitor medication administration practices and the appropriate use of medical equipment. Identify and address medication errors, procedural concerns, and opportunities for improvement. Ensure proper medication storage, documentation, and disposal. Train, Mentor, & Support Our Care Team Conduct nursing-related orientation training for newly hired unlicensed personnel, including facility procedures, medication administration, and resident-specific care needs. Provide continuing education, in-service training, and refresher courses to address regulatory requirements, procedural changes, and identified staff development needs. Conduct initial and ongoing competency evaluations to ensure staff can safely perform assigned responsibilities. Provide additional training, coaching, and competency reassessments when performance concerns or knowledge gaps are identified. Maintain accurate documentation of staff training, education, and competency evaluations. Collaborate with the Clinical Nurse Supervisor to ensure staff remain current with required training and competencies. Provide performance feedback and contribute to annual evaluations of unlicensed personnel. Promote Quality & Compliance Assist the Clinical Nurse Supervisor in maintaining compliance with applicable federal, state, and local regulations. Participate in licensing surveys, regulatory inspections, and corrective action processes as needed. Assist in developing, reviewing, and implementing nursing policies, procedures, and required documentation. Maintain current knowledge of assisted living regulations, nursing, practices, and industry standards. Investigate, document, and report suspected resident abuse, neglect, or maltreatment in accordance with applicable reporting requirements and facility procedures. Maintain resident privacy, confidentiality, dignity, and respect. Identify safety concerns and participate in quality improvement initiatives to enhance resident care and operational effectiveness. Collaborate With Our Team Maintain effective communication and positive working relationships with residents, families, staff, and healthcare providers. Collaborate with the Clinical Nurse Supervisor and leadership team to address clinical concerns and operational needs. Participate in staff meetings, committees, and interdisciplinary discussions. Demonstrate compassion, professionalism, flexibility, and ethical decision-making. Respond appropriately to emergencies, safety hazards, and other urgent situations. Report injuries involving residents, employees, or visitors in accordance with facility procedures. Provide assistance to other departments and participate in facility events as needed.
