Home Health Care, Inc. is hiring a Registered Nurse (RN) Case Manager for a focused North Minneapolis territory covering 55401, 55402, 55403, 55405, 55411, 55412, 55415, 55430, and 55454 . You'll work independently in the field while having an experienced clinical team behind you when you need them. Independence Without Being on an Island Your day won't revolve around a nurses' station. You'll travel between patients, manage your assigned caseload, complete skilled visits, communicate directly with physicians, and make clinical decisions based on what you're seeing in the home. That autonomy comes with responsibility. You're the RN in the room. You'll need to recognize changes, prioritize concerns, communicate clearly, and determine what needs to happen next. But independence doesn't mean doing everything alone. You'll have clinical supervisors and an interdisciplinary team available to collaborate when a patient needs additional support. Why RNs Take a Look $85,000-$130,000+ annual earning potential $6,000 First-Year Bonus Monday-Friday, 8:00 a.m.-5:00 p.m. Focused North Minneapolis territory One-on-one skilled nursing Ownership of an assigned caseload Field-based independence and clinical autonomy Mileage reimbursement between visits Cell phone reimbursement every paycheck Free MedBridge Education Hub Nursing licensure reimbursement Annual scrub reimbursement Comprehensive benefits PTO and paid holidays 401(k) with employer match Experienced clinical and interdisciplinary support More than 30 years serving Minnesota home health patients Your Judgment Matters Here As the RN Case Manager, you'll: Manage an assigned North Minneapolis caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and ongoing skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor chronic and medically complex conditions Recognize and respond to changes in condition Educate patients and caregivers Communicate directly with physicians Collaborate with PTs, OTs, SLPs, social workers, and other clinicians Help prevent avoidable emergency department visits and rehospitalizations Participate in an on-call and weekend rotation Bring the Nurse You've Become We're interested in experienced RNs from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, Community Health, and other clinically complex settings . You'll need an active Minnesota Registered Nurse (RN) license , three or more years of nursing experience , strong assessment and critical-thinking skills, excellent organization, and a valid driver's license with reliable transportation.
