What You'll Be Doing Complete comprehensive nursing visits and assessments Perform wound care and medication management Provide patient and family education Identify changes in condition and intervene proactively Support safe transitions and improved outcomes Collaborate With An Exceptional Team Partner with physicians and interdisciplinary clinicians Communicate changes and recommendations Maintain accurate documentation Help patients remain safe and independent at home What You Bring Required Active Minnesota RN license Reliable transportation and valid driver's license Strong assessment and communication skills Confidence working independently Preferred Home health, hospice, TCU, or post-acute experience Passion for relationship-based, patient-centered care Great Nurses Don't Just Treat Patients.
