If you've worked in home health long enough, you know there is a particular skill to completing a great admission. You walk into a patient's home with a referral, a medical history, and some preliminary information, but it's your job to uncover the full clinical picture. Home Health Care, Inc. is hiring an experienced Start of Care Nurse (RN) to focus on home health admissions throughout the Northern Greater Twin Cities Metro Area . If assessments, OASIS, clinical problem-solving, and meeting new patients are the parts of home health you do especially well, this role allows you to make those strengths the focus of your work. Become an Admissions Expert Not every RN enjoys Start of Care visits. They require strong assessment skills, attention to detail, confidence with OASIS, efficient documentation, and the ability to quickly establish trust with a patient you've never met before. But some nurses are exceptionally good at them. You know how to walk into an unfamiliar situation and quickly determine what's clinically important. You can reconcile medications, identify safety concerns, understand the patient's functional limitations, educate the family, and translate everything you've learned into a strong plan for the clinicians taking over the patient's ongoing care. That means completing a comprehensive assessment, identifying immediate needs, educating the patient and caregiver, building the initial plan of care, and making sure the ongoing team understands what needs to happen next. Then you get to do it again with someone completely different. For nurses who enjoy new patients, new clinical situations, and solving the initial puzzle , admissions can be one of the most interesting areas of home health nursing. What You'll Be Doing Complete comprehensive Start of Care assessments for new home health patients Perform accurate and timely OASIS documentation Complete medication reconciliation and identify potential concerns Assess clinical, functional, environmental, and safety needs Develop individualized plans of care based on patient goals and clinical findings Educate patients and caregivers on medications, disease management, safety, and home health expectations Identify concerns requiring immediate intervention or provider communication Coordinate with physicians and referring providers Communicate a clear clinical picture to the RN Case Manager and ongoing care team Collaborate with PT, OT, SLP, social work, and other disciplines Maintain accurate, timely, and compliant documentation A Great Fit for an Experienced Home Health RN This isn't a role where we'd expect an RN to learn OASIS from scratch. We're looking for someone who already understands home health and can confidently complete Start of Care visits while managing the clinical and documentation requirements that come with them. What You Bring Required:
Active Minnesota Registered Nurse (RN) license Previous home health nursing experience Experience completing Start of Care assessments Working knowledge of OASIS Strong head-to-toe assessment and critical-thinking skills Strong clinical documentation and organizational skills Confidence independently evaluating new patients in their homes Excellent patient, caregiver, and provider communication Valid driver's license and reliable transportation
Preferred:
Strong OASIS proficiency Dedicated home health admissions experience RN Case Management experience Experience with medically complex and post-acute patients If Admissions Are Your Strength, Make Them Your Specialty.
