What You'll Be Doing As a Floating RN Case Manager, you'll provide skilled nursing visits and coordinate care for patients throughout multiple territories. You'll serve as a resource for teams while ensuring continuity, quality outcomes, and exceptional patient experiences.
Responsibilities include:
Performing comprehensive patient assessments. Developing and updating individualized plans of care. Educating patients and families regarding medications, safety, and disease management. Supporting different territories based on patient and staffing needs. Assisting with admissions, routine visits, and case management coverage. Collaborating with therapists, social workers, physicians, and other members of the interdisciplinary team. Completing documentation and OASIS assessments. Participating in rotating on-call responsibilities. Qualifications Active Minnesota Registered Nurse License Required Strong clinical judgment and critical thinking skills. Ability to adapt to different teams and changing priorities. Excellent communication and relationship-building abilities. Home health experience preferred but not required.
