Home Health Care, Inc. is hiring a Registered Nurse (RN) Visit Nurse to provide skilled nursing throughout Maple Grove, Dayton, Elk River, Rogers, Osseo, Otsego, Albertville, Hanover, St. Michael, and surrounding communities . This role gives you an opportunity to focus heavily on hands-on skilled nursing visits while working alongside RN Case Managers and an experienced interdisciplinary team. Come In. Assess. Treat. Educate. Your patients need an experienced nurse. You'll walk into the home, assess what's happening clinically, provide the ordered care, answer questions, educate the patient and caregiver, document what you're seeing, and communicate important changes back to the team. Then you move on to your next patient. It's a different rhythm than traditional RN Case Management. For nurses who enjoy being in the field and want to spend more of their time providing direct clinical care, that distinction matters. Spend Your Day Using Your Nursing Skills You'll:
Provide skilled nursing visits throughout your assigned territory Complete nursing assessments and ongoing evaluations Perform wound care and other ordered treatments Provide medication management and education Monitor chronic and medically complex conditions Support patients following hospitalization Recognize and communicate changes in condition Educate patients and caregivers Support admissions and additional skilled visits as needed Collaborate with RN Case Managers, physicians, therapists, social workers, and other clinicians Maintain accurate and timely clinical documentation Participate in required on-call and weekend rotations Your Clinical Background Can Translate We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, and other hands-on clinical environments . If you can walk into a patient's home, assess the situation, provide excellent skilled nursing, communicate concerns, and confidently work independently, you may be exactly what we're looking for. You'll need an active Minnesota Registered Nurse (RN) license , strong clinical judgment and communication skills, and a valid driver's license with reliable transportation. Home health and previous field nursing experience are preferred. Maybe You Don't Want Less Nursing. You Want More of the Right Parts.
