Twin Cities, St. Cloud & Select Rural Minnesota Communities Make a Difference Beyond the Hospital Are you an experienced RN looking for a position where you can combine clinical expertise, leadership, and meaningful relationships with patients and families? Pediatech Nursing is seeking a compassionate, organized, and dedicated Registered Nurse (RN) Case Manager to join our growing pediatric home health team. As an RN Case Manager, you will play an important role in coordinating exceptional patient care while providing clinical leadership, education, and support to our field nursing staff. You'll work closely with children and young adults with complex medical needs, their families, healthcare providers, and fellow members of our care team. What You'll Do As an RN Case Manager, you will help ensure our patients receive safe, high-quality care while supporting and developing our nursing team.
Responsibilities include:
Conduct in-home nursing assessments for pediatric and young adult clients. Develop, implement, and update individualized plans of care. Supervise and support field nurses to promote high-quality patient care. Facilitate orientation and onboarding for newly hired nurses. Provide ongoing clinical education, training, and support to nursing staff. Ensure compliance with agency policies, procedures, and regulatory requirements. Identify opportunities to improve clinical policies, procedures, and best practices. Coordinate care with physicians, specialists, clinics, hospitals, DME providers, and other members of the interdisciplinary care team. Collaborate with fellow RN Case Managers and office staff to ensure seamless communication and care coordination. Advocate for patients and families while helping coordinate their healthcare needs. What We're Looking For We're looking for someone who is compassionate, organized, dependable, and confident in their clinical skills and who enjoys working both independently and as part of a team. Nursing Experience Minimum of 3 years of nursing experience required 2 years of pediatric nursing experience preferred 2 years of home health experience preferred Strong clinical assessment and communication skills Ability to work independently while effectively collaborating with patients, families, and healthcare professionals Education & Licensure Bachelor of Science in Nursing (BSN) preferred Associate Degree in Nursing (ADN) may be considered with Governing Body approval Current, unrestricted Registered Nurse (RN) license in the State of Minnesota Valid driver's license with a satisfactory driving record Why Join Pediatech Nursing?
