Full-Time | Monday-Friday | 8:00 a.m.-5:00 p.m. +
On-Call & Weekend Rotation Compensation:

We're hiring an RN Case Manager to support patients throughout New Brighton, Fridley, Mounds View, Spring Lake Park, North Minneapolis (55432), St. Paul (55112), and surrounding areas . As the RN Case Manager, you'll use that familiarity alongside your clinical judgment to manage the plan of care and help patients safely recover at home. You'll:
Manage an assigned home health caseload Complete comprehensive nursing assessments Perform admissions, recertifications, and skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor medically complex and chronic conditions Recognize and respond to changes in condition Coordinate with physicians, PTs, OTs, SLPs, social workers, and other clinicians Help prevent avoidable emergency department visits and rehospitalizations Participate in an on-call and weekend rotation Your Experience Can Translate to Home Health We're interested in experienced RNs from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings . You'll need an active Minnesota Registered Nurse (RN) license , three or more years of nursing experience , strong assessment and critical-thinking skills, excellent organization, and a valid driver's license with reliable transportation. Home health, OASIS, and previous case management experience are preferred. Don't Just Wonder What Happened to the Patient. Be There to See It.
