Schedule:
Full-Time | Monday-Friday, 8:00 AM - 5:00 PM | 1 Weekend Per Month | 1
On-Call Shift Per Week Compensation:
$95,000 - $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses

What You'll Do Manage a home health caseload throughout Maple Grove, Dayton, Elk River, Rogers, Osseo, Otsego, Albertville, Hanover, and St. Michael. Complete OASIS assessments and individualized plans of care. Deliver skilled nursing visits and patient education. Coordinate care with physicians and interdisciplinary team members. Help patients remain healthy, independent, and safely at home. Maintain timely and accurate clinical documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical judgment and ability to work independently. Home health experience preferred, but not required. Excellent communication and organizational skills. Reliable transportation for local travel.
