We're hiring an experienced Registered Nurse (RN) Case Manager for a focused St. Paul territory covering 55103, 55104, 55105, 55106, 55108, 55114, and 55116 . You'll build familiarity with the neighborhoods you serve, develop relationships with your patients and families, and take clinical ownership of an established geographic area. You know which patient normally runs a little hypertensive. You know which family member manages the medications. You remember what the wound looked like last week. You recognize when someone's breathing, mobility, cognition, or overall presentation isn't their normal. That familiarity matters. As the RN Case Manager, you'll combine those ongoing patient relationships with the clinical judgment and coordination required to keep medically complex patients safely at home. What You'll Do Manage an assigned home health caseload within your St. Paul territory Complete comprehensive nursing assessments Perform admissions, recertifications, and ongoing skilled visits Develop and update individualized plans of care Complete OASIS assessments and documentation Provide medication management and disease education Manage wounds and other skilled nursing needs Monitor medically complex and chronic conditions Recognize changes in condition and intervene appropriately Educate patients and caregivers Coordinate with physicians, PTs, OTs, SLPs, social workers, and other care team members Help reduce avoidable emergency room visits and rehospitalizations Participate in an on-call and weekend rotation Bring Your Experience Into the Community Strong home health Case Managers come from many different clinical backgrounds. Maybe you've spent years in Med/Surg learning to prioritize competing needs. Maybe ICU or step-down strengthened your assessment skills. Maybe you've worked in transitional care, rehabilitation, skilled nursing, or case management and understand what patients need after they leave a facility. Or maybe you're already a home health RN looking for a stronger compensation package and a focused St. Paul territory . We're interested in experienced nurses from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings .

What You Bring Required:

Active Minnesota Registered Nurse (RN) license Three or more years of nursing experience Strong clinical assessment and critical-thinking skills Excellent patient and caregiver communication Ability to independently manage a productive patient caseload Strong organization and follow-through Valid driver's license and reliable transportation

Preferred:
