What You'll Be Doing As a Floating Visiting Nurse, you'll provide skilled nursing visits while supporting patients and care teams throughout the Twin Cities. You'll help maintain continuity and ensure patients continue receiving high-quality care, regardless of changing census or staffing needs.Responsibilities include:
Conducting skilled nursing visits and comprehensive assessments. Providing wound care, medication management, and patient education. Developing and updating individualized plans of care. Supporting patients and caregivers with disease management and safety. Floating to nearby territories based on operational needs. Collaborating with therapists, physicians, and interdisciplinary team members. Completing documentation and OASIS assessments when required. Participating in limited on-call rotation. Qualifications Active Minnesota Registered Nurse License Required Strong clinical and communication skills. Ability to adapt and work independently. Home health experience preferred but not required.
