Synergy Homecare MSP Metro (office in Roseville) is seeking an experienced Registered Nurse to provide care in clients homes throughout the central metro area. This is a flexible part-time position (approximately 16-20 hours/week).

Duties:

• Assess clients in their homes, create Plans of Care, and provide nursing services
• Assist with caregiver training/education
• Collaborate with clients, family members, care-providers, and co-workers to provide individualized client care
• Provide reliable care and complete timely documentation of all visits and assessments

Qualifications:

• Nursing degree, along with a valid MN registered nursing license
• Strong clinical skills for assessing client needs and providing care
