JOB DESCRIPTION
HealthPartners North Suburban Family Physicians has an exciting opportunity for a Registered Nurse to join our team! This casual position will be based at our Roseville clinic with potential for coverage at our Lino Lakes and Hugo clinics. This casual position will provide coverage at all 3 North Suburban Family Physicians clinics Monday- Friday with start times between 7:45am
- 8:30am and end times between 4:15pm
- 5pm.
Nursing care accountabilities include:
Demonstrates clinical competence in using the nursing process to provide care to patients and their families. Implements established Nursing Policies and Procedures and Standing Orders. Provides emotional, intellectual and psychological support to patients and their families. Documents accurately and completely and keeps record regarding clinical practice. Shares information with other members of the health care team. Makes appropriate patient referrals. Uses interdisciplinary resources to enhance own clinical decision making Provides continuity among home, office, inpatient setting and community. Uses appropriate systems, automated or manual, to obtain patient information or schedule appointments.Care Coordination accountabilities include:
Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention Identifies and secures patient services needed for patient care Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.Education accountabilities include:
Works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals. Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation. Engages in ongoing evaluation of the effectiveness of teaching in terms of understanding and application.Additional responsibilities include:
Participates in patient care conferences, in-services and educational programs. Keeps current on best practices related to nursing and patient care Serves as a role model in the enhancements of nursing practice and professional conduct. Demonstrates flexibility in performing additional responsibilities to contribute to the improvements in patient care and the functioning of the care unit and other support services. Provides support to professional and technical staff. Formulates and attains personal and professional goals which promote individual growth, and support the organization vision, mission and values. Demonstrates ability to assess and problem solve. Serves as a catalyst for improving standards of practice.This position requires:
Graduate of an accredited school of nursing Current RN licensure in Minnesota Current RN licensure in Wisconsin (within two months of hire) Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable. Strongly demonstrated written and oral communication skills Preferred qualifications include: One year full time employment as an RN Triage or Care Management experience within the last three yearsABOUT US
