Your visits may include:

Comprehensive nursing assessments Wound care and other ordered treatments Medication reconciliation, management, and education Post-hospitalization nursing Chronic disease management Patient and caregiver education Monitoring medically complex conditions Recognizing and communicating changes in condition Admissions and additional skilled visits as needed Collaboration with RN Case Managers and physicians Coordination with PTs, OTs, SLPs, social workers, and other clinicians Accurate and timely clinical documentation Participation in an on-call and weekend rotation Bring the Skills You've Already Built We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on clinical environments . You'll need an active Minnesota Registered Nurse (RN) license , strong assessment and critical-thinking skills, confidence working independently, excellent communication, and a valid driver's license with reliable transportation. Home health, wound care, chronic disease management, and OASIS experience are preferred.
