Home Health Care, Inc. is hiring a Registered Nurse (RN) Visit Nurse to provide skilled nursing throughout Minnetonka, Wayzata, Mound, Delano, Watertown, Waverly, and surrounding western Metro communities . Your Workday Has a Windshield You'll start your day knowing which patients need you, then move through the community providing one-on-one skilled nursing visits. One home may require wound care. The next patient needs medication education. Another recently returned from the hospital and needs an experienced RN to assess how they're doing. You still have documentation, clinical responsibility, and productivity expectations. But between patients, you get a change of scenery, a chance to reset, and the independence of working in the field. Road You'll:

Provide skilled nursing visits throughout your assigned territory Complete nursing assessments and ongoing evaluations Perform wound care and other ordered treatments Provide medication management and education Monitor chronic and medically complex conditions Support patients following hospitalization or changes in condition Recognize clinical concerns and communicate them appropriately Educate patients and caregivers Support admissions and additional skilled visits as needed Collaborate with RN Case Managers, physicians, therapists, social workers, and other clinicians Maintain accurate and timely clinical documentation Participate in required on-call and weekend rotations Bring the Experience You've Already Built We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on clinical environments . You'll need an active Minnesota Registered Nurse (RN) license , strong clinical assessment and critical-thinking skills, confidence working independently, excellent communication, and a valid driver's license with reliable transportation. Home health, wound care, chronic disease management, and OASIS experience are preferred. Maybe You Don't Need a Different Career. You Need a Different Environment.
