You'll assess residents, respond to changes in condition, communicate with providers and families, support caregivers, oversee clinical processes, and help make sure residents receive the right care at the right time.

Your responsibilities will include:

Resident assessments and changes in condition Medication management and treatments Individualized service plans and nursing interventions Clinical guidance for caregivers and medication staff Communication with residents and families Coordination with physicians, pharmacies, hospitals, home health, hospice, and other providers Admissions and transitions of care Clinical documentation Minnesota Assisted Living regulatory compliance Shared rotating on-call responsibilities A Different Pace of Nursing You won't be meeting a completely new group of patients every shift. You'll get to know residents, understand their baseline, recognize when something has changed, and build relationships with the people you're caring for. What We're Looking For Active Minnesota Registered Nurse (RN) license in good standing Strong assessment and clinical judgment skills Ability to work independently and provide clinical direction Strong communication and documentation skills Comfort working with older adults and changing clinical needs Ability to participate in every-other-weekend and shared on-call rotations Assisted Living, Memory Care, Long-Term Care, Home Health, Hospice, Hospital, Geriatric, or other relevant nursing experience welcomed Keep Using the License You Worked Hard to Earn.
