Key Responsibilities:

Conduct comprehensive annual home health risk assessments (average 45 minutes each) for Medicare Advantage participants Evaluate prior and current medical history, chronic conditions, and medication usage Educate patients on diagnoses and treatment regimens Identify care gaps and provide actionable recommendations, helping coordinate follow-up where needed Visits may include both annual assessments and post-discharge follow-ups No requirement for developing treatment plans or prescribing medication

Qualifications:
